I was thinking it was some sort of birth control device, but it looks new. & would be very painful. If you screwed that into your hand, eyelid, cheekbone, jaw, finger, toe, ear, for a while, you wouldn't want to screw anything else. So, voila'...birth control.
Sort of looks like it would be used for lengthening something. Bones, maybe? But I don't get the crossbar on the top, so maybe not. But it looks like as you would twist the screws the distance would become greater between the rings. Hmmm.
It's a nipple trainer. The bar goes through a piercing and you stretch your nipple over time to make it stand up and become lots longer. Seriously, that's what it is.
Can you imagine the inventor going through prototypes to adjust and tweak it (pun intended) to get it just right? I shudder to think what his/her own nipples look like.
File this under "too much information", but I have a nip piercing and wearing a shield with a barbel is too much pain for me...I can't conceive of any sane reason why someone would need or want to stretch their nipples any longer than they were intended to stretch. Same with ear lobes, frankly. Or lips or nostrils. Stabbing a 14 gauge steel bar through them is entirely another story ; )

If you have inverted nipples, it's a good idea to do what you can to get them to stand out further so it is easier for your baby to latch on. Some babies literally have to feel the nipple rubbing on the roof of their mouths, or they continue to scream with their mouth wide open, on the breast. That said, I don't think this device has anything at all to do with anything nursing-related.
